The Diabetes Care Center (DCC) understands the concerns and difficulties related to diabetes and is dedicated to providing the management, support, and education needed to help control the progression of the disease. Recognized by the American Diabetes Association, SACMC's program is designed to assist and instruct insulin-dependent, non-insulin-dependent and gestational diabetes patients of all ages.
The DCC uses a multidisciplinary team approach with the patient's personal physician providing a referral and follow-up care.
A registered nurse, who is certified in diabetes care, provides the patient with instructions in self-management of the disease and a registered dietician assesses the nutritional status and assists with developing an individual meal plan. To maximize the positive results of treatment an individualized treatment plan is established for each patient.
