I would like to take this opportunity to welcome each of you to Aetna’s Community Choice health plan and introduce you to the services and affiliated providers of San Angelo Community Medical Center (SACMC). The information that follows is intended to familiarize you with the organization and provide you additional resources that may be helpful to some if any transitional care assistance is required.
Our goal is to make any transition period as easy as possible for you. With a full complement of services and providers, we will help you locate the appropriate healthcare provider or service for you or your family's healthcare needs. To assist in this process, Aetna is providing an informational line (855-569-5723; Monday-Friday, 8am to 6pm) to help you find a provider. To further assist you, SACMC has a dedicated implementation specialist for the City of San Angelo (COSA), Randall Clinkinbeard, who is available for further questions at 325-947-6987. Other large affiliated provider groups such as West Texas Medical Associates (WTMA) and Community Medical Associates (CMA) are also committed to transitional excellence and have each dedicated a patient advocate/access line (WTMA: 224-5674; CMA: 245-4262).
On this page, you will find Aetna’s list of providers, convenient maps and a quick reference guide. In addition, we have included a medical release document that may be helpful if any health information is required to be transferred between providers.
