Patient Relations/Complaints

Patient Complaints/Protective Services:

Because your care is important to the hospital staff, we have established a patient relations/guest relations improvement program along with an active Performance Improvement program. Problems with your care should be reported to the charge nurse, patient care manager or director of the department providing the service.

If the problem is not resolved to your satisfaction, please contact Cindy Tschudi, Risk Manager, at 325-947-6468 or download our complaint form and fax to 325-947-6872. The hospital will review your complaint and take action. We strive to provide you the best possible care.

Advance Directives:

Your rights as a patient.
Every adult has the right to decide what may be done in the course of medical treatment. Directives help patients make informed and rational decisions about their course of treatment. Under Texas law, an advance directive allows you to make your wishes known concerning medical treatment before you actually need such care.

Directives:

  • Living Will or Directive to Physicians allows you to instruct your physician to use, or not to use, artificial methods to prolong your life if you are terminally ill.
  • Medical Power of Attorney for Health Care designates someone you trust as your agent to make health care decisions on your behalf in the event you are unable to make such decisions.
  • Declaration for Mental Health Treatment gives direction of your desires for medication and treatment.

Ethics Committee:

This committee shall serve as an advisory body when medical/ethical issues involving the clinical care of patients are encountered by staff, physicians, patients and families. Information about and/or access to the committee may be obtained through the unit charge nurse, patient care manager, director or adminstrative supervisor.
